Monthly:  By Selecting this option your credit card will be charged monthly for the total number of seats multiplied by the per seat cost.  For example, if 12 seats are selected, your monthly cost will be $238.80 (12*$19.90 per seat)

DVP Platinum Elite:  10-20 $19.90 per seat

DVP Platinum Elite:  21-45 $17.98 per seat

DVP Platinum Elite:  46-70 $12.99 per seat

Quarterly:  By Selecting this option your credit card will be charged quarterly for the total number o seats multiplied by the Quarterly cost per seat which is discounted at 5%.  For example, if 12 seats are selected your quarterly cost will be $680.76 (12*18.91=$226.92*3 months=$680.76)

DVP Platinum Elite:  10-20 $18.91 per seat

DVP Platinum Elite:  21-45 $17.08 per seat

DVP Platinum Elite:  46-70 $12.34 per seat

Annual:   By Selecting this option your credit card will be charged one time annually for the total number of seats multiplied by the Annual cost per seat which is discounted at 10%.  For example, if 22 seats are selected, your annual cost will be $4271.52 ($16.18***=$355.96*12=$4271.52).

Payment Terms: This service is a monthly subscription-based model with an annual auto renewal. The credit card will be charged on the 5th day of each month. Should the 5th of the month not fall on a business day, payment will be executed on the business day immediately prior to the 5th. If opting for the annual payment option, your card will be charged the 5^th day of the month following receipt of the subscription form for the annual payment amount.  The renewal will occur 1 year following the subscription date.  This service has a 1-year rate lock from the date of subscription. If the subscription level changes the applicable upgrade fees will be assessed on the transaction the 5^th of the month following the upgrade.

Assurance: By signing the below, I as the customer, certify I am an authorized user of the credit card provided and as such, there will be no disputed for these scheduled transactions with the bank or credit card company; so long as the transactions correspond to the terms indicated in this subscription form. In addition, as the customer, I will provide Broker Builder Solutions with updated payment information promptly. Failure to comply may result in a service interruption.

Professional Liability Insurance: Broker Builder Solutions LLC shall obtain and always maintain during the prosecution of the work under this Agreement Professional Liability insurance to include Errors and Omissions and Cybersecurity.

Termination: Broker Builder Solutions or Customer may terminate this Agreement by providing thirty (30) days written notice (“Termination Notice”) to the other party. Cancellation requests should be submitted via this Termination Form.  Billing will cease the billing cycle following or coinciding with the 30-day period.  Example, if the termination is received on the 10th of February, billing will be complete on March 5^th and cease with the April billing.  If the termination is received on February 1^st, the final billing will be February 5^th.
