Termination: I understand that submission of this form will operate as my thirty (30) days written notice (“Termination Notice”) to end my Data Validation Platform subscription. A copy of this form will be emailed to both the individual submitting the request and the Data Management Solutions finance team. I understand that billing will cease with the billing cycle following or coinciding with the 30-day period. Example, if the Termination Notice is received on the 10th of February, billing will be complete on March 5th and cease with the April billing. If the Termination Notice is received on February 1st, the final billing will be February 5th. Data Management Solutions will inactivate my subscription coinciding with the Termination Notice and will send an email confirmation.
